Think Bullish
Chiropractic SEO

Most chiropractors rank #1 somewhere and are invisible across town.

A single citywide ranking hides it. We scan the whole service area, page by condition and street by street, and fix the squares where patients who would have driven to you are being shown somebody else.

Measured across 7 practices

“We rank #1” and “patients can find us” are different claims.

Across 77 tracked chiropractic keywords, 60 hold the top spot somewhere on the grid — and the average practice appears in the map pack at just 11.3% of the 81 points we scan. Both things are true at once, and only one of them shows up in a normal ranking report.

A disc and nerve pain practice — “Spinal disc solutions”, scanned August 30, 2026. Each square is one search point, 1 mile apart, spanning 8 miles edge to edge.

The grey squares are the whole opportunity. A practice sitting at #1 outside its own front door and nowhere four miles away is not losing on reputation or on reviews — it is losing on proximity signals, service-area content and local authority, all of which are fixable.

The answer layer, on chiropractic queries

Google now answers the patient before your listing loads.

We track 56 real questions chiropractic patients type — sciatica, disc pain, decompression, injury care — and check what the answer engines say back.

Zero, today. Being named is not being linked, and we would rather show you that than round it up.

56 non-brand patient questions across 7 chiropractic and spine practices, scanned August 29, 2026 to September 4, 2026. Brand-navigational queries are excluded: being named when someone searches your own clinic by name is a different result.

The work

Built around how spine patients actually search.

01

Condition pages, not a services list

Patients search the thing that hurts: sciatica, a bulging or herniated disc, spinal stenosis, neck pain after a car accident. Each of those is its own page with its own candidacy, cost and what-to-expect answers — because each is a different decision, and one 'Services' page cannot rank for all of them.

02

Map-pack coverage across the drive radius

Ranking in the pack at the clinic's own address is the easy part. We scan a grid across the whole service area and work the squares where the practice drops out, because that is where the patients who would still drive to you are being handed to someone else.

03

Personal injury and attorney-referral search

Auto-injury cases behave nothing like a wellness adjustment: different queries, different urgency, and a referral network searching alongside the patient. It gets its own pages and its own local signals rather than being folded into general chiropractic.

04

E-E-A-T that survives a health update

Chiropractic is YMYL, so Google holds it to a higher trust bar than most local business categories. Named authorship with real credentials, provider bios that connect to the conditions they treat, honest sourcing, and review ownership — the signals that decide whether a health page keeps its rankings through a core update.

05

Eligibility to be the cited answer

An AI Overview now answers a majority of the patient questions we track, and it names practices before anyone scrolls. Entity clarity, schema, and directly answerable content decide whether a practice is in that answer or invisible above the fold.

06

Reporting you can argue with

The grid, the tracked question set, and Search Console — with the windows and dates on them. Every number on this page comes out of the same reporting a client sees, which is the only reason we are willing to publish the unflattering ones.

Chiropractic SEO questions, answered properly.

Why does my practice rank #1 when I search, but patients still cannot find us?

Because you are searching from inside your own clinic. Map-pack results are re-ranked by the searcher's location, so the practice that owns position one at its own address routinely disappears three or four miles away. Across the chiropractic practices we measure, most hold #1 somewhere on the scan grid while appearing in the pack at only about a tenth of the points across their service area. Searching from your own desk — or checking a single citywide ranking — is the one test guaranteed to hide the problem.

How long does chiropractic SEO take to work?

Technical and local fixes can move within weeks: correcting a Google Business Profile category, service-area content, or an indexing fault often shows up in the map grid inside one or two scan cycles. Competitive condition rankings take longer, usually several months, because they depend on content that has to be written, indexed and then earn trust. Anyone quoting a fixed timeline before seeing the site's current technical state, authority and market is guessing.

Do A.I. Overviews mean SEO no longer matters for chiropractors?

It means the target moved. Across the chiropractic patient questions we track, roughly six in ten now return an A.I. Overview, and being named inside that answer is a different job from ranking beneath it. The underlying signals are largely the same ones — entity clarity, structured data, genuine expertise, local authority — but eligibility to be quoted has to be worked and measured deliberately. Traditional rankings still matter; they are no longer the whole result.

Can you guarantee first-page rankings?

No, and neither can anyone else. Nobody controls Google's ranking systems or which sources an answer engine selects. What can be committed to is the work and the measurement: the technical corrections, the condition and location coverage, the local signals, and reporting that shows grid coverage, tracked question visibility and Search Console performance with the dates and windows attached, so progress is arguable rather than asserted.

Is SEO worth it if the practice already runs Facebook ads?

They capture different demand and neither replaces the other. Paid social creates demand among people who were not looking; search captures people already in pain and actively choosing. Practices that run both point them at the same offer and booking process. The practical difference is that ad performance stops the day spend stops, while the map coverage and condition pages built by SEO keep producing — which is also why SEO is judged over quarters rather than weeks.

What is the difference between chiropractic SEO and local SEO?

Local SEO is the map-pack half: Google Business Profile, proximity and service-area signals, citations, reviews. Chiropractic SEO includes that and adds the condition-level organic work — sciatica, disc, decompression and injury pages that answer candidacy, cost and what-to-expect questions. Most practices need both, because the map pack wins the person searching "chiropractor near me" while the condition pages win the person searching what is actually wrong with them.
